How to claim your rebate
- Qualify. Visit us for a contact lens fitting, then purchase a minimum of 2 boxes of a qualifying WaterInnovations™ contact lens in a single transaction. Purchases must be from the eye care practitioner who prescribed your lenses.
- Gather your documents. Within 60 days of purchase, have electronic copies of your sales receipt, your eye exam / lens fitting receipt, and the UPC from your contact lens box.
- Submit online. Complete the claim form at AlconChoice.com (offer code EXAC - 26H2) and choose a physical or virtual Alcon Prepaid Visa card.
